In the child with DIVA, even outside the emergency-urgency situations, a device for peripheral venous access can be positioned with relative ease by resorting to the ultrasound-guided puncture and cannulation of deep veins of the arm or forearm.
Different peripheral access devices can be used for this purpose: short peripheral cannulas, long peripheral cannulas (mini-midlines) and traditional midlines.
In practice, however, short cannulas (< 6 cm) inserted by ultrasound guidance into deep veins of the upper limb tend to have a very short duration (often not reaching 24 hours in duration) and are associated with a high risk of dislocation and extravasation.
On the other hand, traditional midlines (> 15 cm) are expensive and have more indication in the extra-hospital setting.
The safest and most cost-effective alternative in these cases is therefore represented by mini-midlines (long peripheral cannulas, 6-15 cm).
